To create and manage your forms, you will need to login with your ISS username and password.
Note: If you would like to create a form that allows file uploads, please see the restrictions and instructions regarding this facility.
To logout of the ISS Form Builder, you must close your web browser.
Once you have created it, there are a number of ways to make your form public so that people can fill it in.
Forms and their associated data are attached to the username of their creator. For corporate use, in order to be able to share your forms with other members of your team, it is advisable to use a team login ID (or role account). If you don't currently have a role ID, you can apply for one at ISS Registration.
Using a role ID is also important if you want to use University corporate branding on your form.
Student societies should also use their non-personal IDs.
